-Birth Registrations-

NEW!!!!  Effective March 30, 2009

For babies born after March 30, 2009 parents may now complete the “Statement of Live Birth” on-line and electronically submit it directly to ServiceOntario’s Office of the Registrar General.   Births occurring in the City of Orillia will no longer be registered with the municipality.  Instead, parents are encouraged to use the Newborn Registration link on the Service Ontario website. This new service allows parents to electronically submit the Statement of Live Birth and submit applications for a birth certificate and social insurance number at the same time.  If you have any questions, please contact Helen Raposo at the City of Orillia (705)325-2310.

For babies born before March 30, 2009, please follow the steps below:

The first step to give your new baby his or her legal identity is to register the birth of the child.  Use the “Statement of Live Birth” form provided in the hospital information package  or pick one up at the Orillia City Centre – 3rd Floor Reception Counter.

You may also register your baby’s birth by using the Online Newborn Registration Service found at www.serviceontario.caThis service allows you to record and print the Statement of Live Birth from the online form.  You must PRINT THIS FORM AND PROVIDE IT TO OUR OFFICE.  This online service will also provide information to create a Social Insurance Number and Birth Certificate for the child once the Registration process is completed.

 

However if you choose to fill out the form, it must be provided to our office for processing, along with a $15.00 administration fee within 30 days of the birth.  (In person - cash or debit, or by mail - cheque or money order made payable to the City of Orillia)  If you require further information, please contact: Helen Raposo, Vital Statistics Co-ordinator at (705) 325-2310.

 

NOTE:  THIS FORM IS A PERMANENT LEGAL RECORD OF YOUR CHILD’S BIRTH – PLEASE READ THE INSTRUCTIONS AND FILL THE FORM OUT CAREFULLY.

After the baby has been registered at our office, the forms are forwarded to the Office of the Registrar General (ORG) for processing.  This step may take from 6 to 12 weeks. When this process is completed, the ORG will mail out an acknowledgement. 

The second step is to order a birth certificate from the Province which is applied for separately.  Application forms can be picked up at the Orillia City Centre - Main Floor Counter or 3rd Floor Reception.  Birth Certificate application forms can also be obtained online from the Service Ontario website at: www.serviceontario.ca

-Marriage Licences-

Marriage Licence Requirements >>

A Marriage Licence is only issued to either or both of the applicants getting married.

Marriage licences are valid for 90 days from the date issued.

Both applicants must sign the application.

Both applicants must provide valid and current proof of age, and government-issued photo identification (eg. birth certificate and driver’s licence, OR a valid passport – please call ahead if you have questions)

Applicants who have been:

divorced in Canada must provide the original certificate of divorce or original decree absolute (or a certified copy of the same) from the Court you were divorced in.  Please note: A divorce order or judgment is NOT acceptable.

divorced outside of Canada must follow the guidelines of the “Out of Country Divorce Package”.  PLEASE ENQUIRE AT THE 3RD FLOOR RECEPTION COUNTER FOR MORE INFORMATION.  This step also requires the applicant to provide an original certificate of divorce or original decree absolute (or a certified copy of the same).

16 and 17 year-old applicants must have the consent of both parents – PLEASE ENQUIRE AT THE 3RD FLOOR RECEPTION COUNTER FOR MORE INFORMATION

Marriage Licence Fee:  $115.00 (cash or debit at time of issue)
