-Birth Registrations-

NEW!!  Effective March 30, 2009, the City of Orillia will no longer administer the Birth Registration process for babies born within the City of Orillia.  ALL birth registrations are now processed by the Office of the Registrar General. 

Parents may submit their registration using one of the following two methods:

Mail-In:  The “Statement of Live Birth” form can be picked up at the City of Orillia, 50 Andrew Street South, 3rd floor. Submit the completed form to The Office of the Registrar General, Box 4600, 189 Red River Road, Thunder Bay, ON  P7B 6L8

Electronically:   www.ServiceOntario.ca/newborn
The Newborn Registration Service is a convenient way to complete up to three important transactions online at the same time:

  1. Register the birth of your baby under the age of one year * (complete Statement of Live Birth form)
  2. Apply for your baby's birth certificate
  3. Apply for your baby's Social Insurance Number

* For births over the age of one year, call The Office of the Registrar General at 1-800-461-2156, to obtain a Delayed Statement of Live Birth form.

The City of Orillia does not accept or process birth certificate applications. The application form must be submitted to the Provincial Government Office of the Registrar General, Ministry of Consumer and Business Services, P.O. Box 4600, 3rd Floor, 189 Red River Road, Thunder Bay, Ontario P7B 6L8.    Forms are available at the City of Orillia, 50 Andrew Street South.

-Marriage Licences-

Marriage Licence Requirements >>

A Marriage Licence is only issued to either or both of the applicants getting married.

Marriage licences are valid for 90 days from the date issued.

Both applicants must sign the application.

Both applicants must provide valid and current proof of age, and government-issued photo identification (eg. birth certificate and driver’s licence, OR a valid passport – please call ahead if you have questions)

Applicants who have been:

divorced in Canada must provide the original certificate of divorce or original decree absolute (or a certified copy of the same) from the Court you were divorced in.  Please note: A divorce order or judgment is NOT acceptable.

divorced outside of Canada must follow the guidelines of the “Out of Country Divorce Package”.  PLEASE ENQUIRE AT THE 3RD FLOOR RECEPTION COUNTER FOR MORE INFORMATION.  This step also requires the applicant to provide an original certificate of divorce or original decree absolute (or a certified copy of the same).

16 and 17 year-old applicants must have the consent of both parents – PLEASE ENQUIRE AT THE 3RD FLOOR RECEPTION COUNTER FOR MORE INFORMATION

Marriage Licence Fee:  $115.00 (cash or debit at time of issue)
